CorSolutions chosen to manage Great-West Healthcare's Chronic Disease Management Program
ROSEMONT, IL, April 27, 2004—CorSolutions Inc., the nation’s leading provider of customer-centric health intelligence and solutions, was named by metro Denver-based Great-West HealthcareSM, a premier national employee benefits provider, to manage its chronic disease management program, which won the Disease Management Association of America’s award for America’s Best Disease Management Program for a National PPO for 2002-2003. Its program includes coronary artery disease, congestive heart failure, diabetes, chronic obstructive pulmonary disease and asthma.
CorSolutions’ disease managers will work collaboratively with the medical management staff in Great-West Healthcare’s regional offices. The team will incorporate data analysis, risk assessment and predictive modeling. It will also assimilate evidence-based medicine, program interventions, and detailed outcomes measurement to further improve quality of care and enhance the productivity of members enrolled in the program.
